RN - Integrated Healthcare Coordinator (Trinidad CO)

QUALIFICATIONS: Associates or Bachelors Degree in Nursing and Colorado Registered Nurse licensure in good standing required. Experienced and skilled in gathering diagnostic information to ascertain cause and effect information pertaining to both medical and behavioral symptoms, mitigations for problems, and solutions to issues. Valid Colorado drivers license and access to reliable transportation during work hours are required. Feel comfortable and oriented in working in behavioral and primary care settings. Conceptualize how a behavioral health professional can help in a wide variety of primary care cases. The person in this position needs to have the skills and the patience to build collaboration among multiple entities.
EXPERENCE: Minimum 1 year of job related experience required
OTHER: Ability to speak Spanish desired but not required. Must be a self-starter, self-motivator and possess exceptional time management and electronic documentation skills. Excellent organizational skill is a must. Must have the skills to help in a wide variety of primary care cases and the knowledge to refer and link clients to center services as well as other medical and human services. Must have excellent computer skills and be willing to enhance skills. Candidates need exceptional time management and documentation skills. Excellent customer and public relations skills are necessary. Have the ability to work on a team, as well as to work independently as needed. Candidate should be prepared to participate as a medical and behavioral health professional for the Patient Centered Medical Home Model.

RESPONSIBILITIES: Under the supervision of the Care Coordination Director, the Healthcare Coordinator will:
Assist patients in problem solving potential issues related to the health care system, financial or social barriers;
Coordinate care with internal and external providers to ensure quality care and follow through ;
Build professional relationships and work closely with medical partners in the community;
Maintain ongoing tracking and appropriate documentation on cases;
Collaborate with community agencies to provide resources;
Provide behavioral health outreach to specific populations;
Be the point of contact for partnering agencies in the community to collaborate with on specific needs of entire population;
Collect and interpret data on a regular basis to determine focus and direction of individual patient needs;
Participate in multidisciplinary staffings;
Conduct all professional activity in an ethical manner, maintain records accurately, adhere to timelines in accordance with Centers standards, follow applicable policies and procedure, consult internally and externally with regard to patient care;
Adapt to change in the workplace and use change as an opportunity for innovation and creativity; and
Perform other duties as assigned.

PHYSICAL DEMANDS:
Light - Exerting up to 20 lbs. occasionally, 10 lbs. frequently, or negligible amounts constantly; requires walking, standing and sitting to significant degrees.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
